How could one tell a doctor was high from examining a patient?

Wouldn’t it be the same for this case too that the patient could be in the wrong like you are a female doctor and have to examine a man patient’s privates for lumps and someone the patient gets hard, maybe because the patient couldn’t help it for some reason?
